Enrollment Forms for Certain Evening Courses

BGS Capstone Course Forms
Seniors in the bachelor of general study degree program are required to complete a capstone course before graduating. The course is individually tailored to each student and a faculty member directs the scope of the course. It can be taken as a writing intensive course (WI) or a non-writing intensive course depending on whether the student needs it to be counted toward the WI requirement for graduation. Your adviser will help you determine if you need the WI version to satisfy graduation requirements.

If you are taking your capstone as a writing intensive (WI) course, you will need the following forms:
Course Approval Form (.doc)
Guidelines for Writing Intensive Courses
(.PDF)
Instructions for filling out the online WI New Course Proposal Form (.doc)

To enroll in the capstone course as a NON-writing-intensive course, complete the following form:
Course Approval Form (.doc)

Medical Terminology (HTH PR 2190) Forms
Because this course requires several online exams, ALL University credit students will need to arrange a proctor for their exams. Students will take FOUR exams online, so proctors will need to have computer access. Information about selecting a proctor and an application to become a proctor can be found in the following downloadable documents:
Tips for selecting a proctor (.doc)
Proctor application (.pdf)

For more information about proctors, visit the MU Direct proctor information page.
