Create New Account - Step 1

When you choose the LOGIN button from FDA Industry Systems you will arrive at the Login page (Figure 1) from which you can login, or Create a New Account. (This page also links to a menu of instructions and tutorials.) Select the "Create New Account" button.

This will take you to the Create New Account screen (Figure 2).

Indicate which systems you will need to access using this Account. (You will only be able to select currently available systems, even though other systems may be displayed. You will be able to add these systems to your account as they become available.)

Fields Included in this Section

Food Facility Registration Module An online program that allows food facilities to register with FDA in accordance with the Bioterrorism Act of 2002.
Low-Acid Canned Food Coming January 2004
Prior Notice An online program that allows anyone with knowledge of the required information to submit notice to FDA of food being imported into the United States prior to its arrival in accordance with the Bioterrorism Act of 2002.

Create New Account - Step 2

Enter the name, address, and telephone information for the person in charge of this Account (See Figure 3), and indicate your understanding of the Terms of 18 U.S.C. 1001, which are as follows:

Under 18 U.S.C. 1001, anyone who makes a materially false, fictitious, or fraudulent statement to the U.S. Government is subject to criminal penalties.

To complete this form do the following:

  1. Complete all required fields and additional fields as desired.
  2. Read the terms of 18 U.S.C. 1001 and select I understand.
  3. Select Continue.

Fields Included in this Section

All fields marked with an asterisk ( * ) are required.
If you are providing a preferred mailing address, the fields marked with asterisks ( ** ) are required.

*First Name The first name of the person who will manage this account.
Middle Initial The first initial of the middle name (if applicable) of the person who will manage this account.
*Last Name / Surname The last name of the person who will manage this account.
*Job Title The professional title of the person who will manage this account; for example, President, CEO, or Plant Manager.
*Company Name The company with which the account holder is associated.
*Password The password that you want to use to login to FDA Industry Systems. Passwords must meet ALL of the following requirements:
  • At least 8, but no more than 32 characters.
  • At least one UPPERCASE letter.
  • At least one lowercase letter.
  • At least one special character: ~ ! @ # $ % ^ * ( ) _ - + = { } [ ] | : ; " , ?. Do not use < >& or '.
  • At least one number.
Some examples of valid passwords would be Yellow$1, December+25 or *Light34. Note: to keep your data secure, do not use these examples as your password.

*Confirm Password The password must be entered a second time for verification.
*Phone Number: Country Code For foreign addresses, the three-digit country code of the telephone number of the account holder.
*Phone Number: Area/City Code The three-digit area code (for domestic addresses) or city code (for foreign addresses) of the telephone number of the account holder.
*Phone Number: Phone Number The telephone number for the account holder.
Phone Number: Extension The telephone extension, if any, dialed after the telephone number, for the account holder.
FAX Number: Country Code The three-digit country code of the telephone number of the FAX machine of the account holder.
FAX Number: Area/City Code The three-digit area code (for domestic addresses) or city code (for foreign addresses) of the telephone number of the FAX machine of the account holder.
FAX Number: FAX Number The telephone number of the FAX machine of the account holder.
E-mail Address The account holder's internet electronic mail address.
Confirm E-mail The internet electronic mail address must be entered a second time for verification.
*Secret Question Choose a question from the drop-down list that you will be asked if you forget your password.
*Secret Answer Your answer to the Secret Question.

Physical Address (Business) of Account Holder

*Country The country in which the account holder is located.
*Address Line 1 The address of the physical location where the account holder is located. This may be different from the mailing address.
Address Line 2 Continuation of the address of the physical location where the account holder is located. This may be different from the mailing address.
*City The city in which the account holder is located.
State For domestic addresses the state in which the account holder is located. A state or a Province / Territory is required.
Province / Territory For foreign addresses, the province, or territory in which the account holder is located. Select the link to choose a Province or Territory from a list for the Country selected. A state or a Province / Territory is required.
*Zip Code (Postal Code) The Zip Code (domestic) or Postal Code (foreign) in which the account holder is located.

Preferred Mailing Address for Issues Regarding this Account (if different)

**Country The country in which the account holder receives mail.
**Address / P.O. Box Line 1 The address at which the account holder receives mail.
**Address Line 2 Continuation of the address at which the account holder receives mail.
**City The city in which the account holder receives mail.
State For domestic addresses, the state in which the account holder receives mail. A state or a Province / Territory is required.
Province / Territory For foreign addresses, the province, or territory in which the account holder receives mail. Select the link to choose a Province or Territory from a list for the Country selected. A state or a Province / Territory is required.
**Zip Code (Postal Code) The Zip Code (domestic) or Postal Code (foreign) in which the account holder receives mail.
*18 U.S.C. 1001 Statement Under United States Code Title 18, Section 1001, anyone who makes a materially false, fictitious, or fraudulent statement to the U.S. Government is subject to criminal penalties. Confirmation that the account holder understands this is required.

Create New Account - Step 3

Review the information you entered for this Account (see Figure 4).

NOTE: The use of names, companies, cities, and countries are used fictitiously for the purpose of illustrating examples of how to complete the screens. Any resemblance to actual persons, living or dead, business establishments, events, or locales is entirely coincidental.

If the information you entered is correct, select Submit. If the information you entered is NOT correct, select Modify to return to the edit screen and make needed changes.

Note: The message, Warning: This address could not be validated, means that the system was unable to verify the address(es) you entered. You are advised to re-check the addresses entered. If they are correct, disregard the notice and submit the application. The application will be processed normally.

Create New Account - Step 4

The status of your request to create an Account is displayed.

If your account creation was successful, the following message will be displayed: You have successfully created an Account. Your account ID is... and a button to login to the FDA Industry Systems home page (e.g. Figure 5).
