Abstract:

This newsletter provides a description of the Chronic Care Model (CCM) for people living with HIV/AIDS (PLWHA), and summarizes features that distinguish HIV/AIDS from other chronic illnesses. It describes components of the CCM, outlines self-management interventions for PLWHA, and offers feedback from clinicians regarding utilization of the CCM for PLWHA. The newsletter includes figures, tables, and references.
