File a Consumer Fraud Complaint

For your convenience, we are pleased to make available a consumer complaint form on our web site. If you are considering filing a complaint, we suggest you review our "How to Complain Effectively" brochure. Make an attempt to first settle your complaint directly.

You may file a written complaint online or download the file, print it, complete it and mail it to the Attorney General's Consumer Protection Division. In order for us to pursue your complaint, you must include your full name, address and phone number. You must also mail in photocopies of any documents that may relate to your complaint claim (contracts, advertisements, correspondence, proof of payment, etc.) Please alert us if you have electronically filed a complaint when you mail in the materials.
