If you have lost your Hunter Education Card, please send us the following information and we will send you a duplicate:

-Name

-Birthdate

-Current Address

-Previous Address (when the course was taken)

-Approximate date and location of the Hunter Education class you took

-$5 (cash or a check made out to the Ky. Department of Fish and Wildlife)
