The questionnaires were used in:

Yuba County, California, to study the effect of pet ownership on household evacuation behavior, pet evacuation failure and pet rescue attempts during an evacuation from a flood. 

Weyauwega, Wisconsin, to study the effect of pet ownership on pet evacuation failure and pet rescue attempts after a hazardous chemical spill due to a train derailment.
