Our Mission

To provide public health leadership to improve cardiovascular health for all, reduce the burden, and eliminate disparities associated with heart disease and stroke.

Heart disease and stroke are, respectively, the first and third leading causes of death in the United States. They are the principal causes of cardiovascular disease death and are also major causes of disability.

Heart Disease and Stroke Prevention Programs

  • A Public Health Action Plan to Prevent Heart Disease and Stroke
    The purpose of the Action Plan, released in April, 2003, is to chart a course for the Centers for Disease Control and Prevention (CDC) and collaborating public health agencies, with all interested partners and the public at large, to help promote and achieve national goals for preventing heart disease and stroke over the next two decades—through 2020 and beyond. The National Forum for Heart Disease and Stroke Prevention is working to implement the Action Plan through the efforts of seven implementation groups.
     
  • State Heart Disease and Stroke Prevention Program
    In 1998, the U.S. Congress provided funding for CDC to initiate a national, state-based heart disease and stroke prevention program with funding for eight states. Currently, 41 states and the District of Columbia are funded, 28 as capacity building programs and 14 as basic implementation programs.
     
  • Paul Coverdell National Acute Stroke Registry
    CDC's state-based program to measure, track, and improve the delivery and quality of stroke care is currently operating in four states.
     
  • WISEWOMAN
    WISEWOMAN's mission is to provide low–income, under– or uninsured 40– to 64–year–old women with the knowledge, skills, and opportunities to improve diet, physical activity, and other lifestyle behaviors to prevent, delay and control cardiovascular and other chronic diseases.
