DMAP outreach sites

 

Providers of direct medical assistance services may contract with the Department of Human Services (DHS) Division of Medical Assistance Programs (DMAP) to help enroll patients in the Oregon Health Plan (OHP) or provide information about the Family Health Insurance Assistance Program (FHIAP). This gives both the patient and the health care facility assurance that they have applied for insurance coverage for the proposed services.

  • Complete an application form (DMAP 6600)
  • Qualified applicants will receive a contract to complete

  • Additional statewide trainings:
    • April 18, 2008, 1:00 to 4:30 - Seaside - Canceled
    • May 7, 2008, 1:00 to 4:30 - Portland
    • May 28, 2008, 1:00 to 4:30 - Beaverton
