The current version of this form is not a modifiable form. You must print the form and return it by mail to:

Or, if you have access to fax, you can fax it to 301-497-5717, or, if you can scan it, and wish to send it as an attachment to an e-mail, send it to bbl_permits@usgs.gov.

Please note that all regulations regarding bird banding permits are described in the Code of Federal Regulations (50 CFR 10 General Provisions, 50 CFR 13 General Permit Procedures and 50 CFR 21 Migratory Bird Treaty Act).

This form may be used to apply for a Master banding permit, apply for a new Subpermit or renew an existing Master permit (subpermittees do not need to submit a renewal form). Please be sure to sign the application form on the last page.
