MDCP Application Kit 2008

The most important part of the application kit is the notice of Federal Funding Opportunity (FFO) for 2008. A notice announcing the FFO was published in the Federal Register. The FFO describes application requirements and format, specifically:  (1) Executive Summary, (2) Background Research, (3) Project Description, (4) Credentials, (5) Finance and Budget, (6) Forms, and (7) Appendices.  The  application kit below covers only instructions, samples, and forms regarding application sections (5) and (6), Finance and Budget, and Forms.

There are basically two steps required to submit your MDCP application online. First, use the application kit below as a guide in developing your application. Convert or scan your application to pdf file format with sequentially-numbered pages. Second, .send your application as an e-mail attachment to Brad.Hess@mail.doc.gov. Check to ensure that your attached application was transmitted successfully. We send confirmations via e-mail within one business day.

Some outgoing e-mail systems might have difficulty with large attachments. If you experience difficulty, split your application into segments and attach each to a separate e-mail. Number each segment (e.g. 1/3, 2/3, 3/3) so that we will know when your completed application has been submitted. We must receive the entire application via e-mail by 5:00 p.m. EDT on July 31. Alternatively, you may choose to submit your application as hard copy. Instructions are included in the FFO.

Do not attempt to transmit all or part of your application via Grants.gov or FedGrants.gov. These online sites are not currently able to transmit certain required portions of MDCP applications. Plan to submit your application via e-mail and/or send five hard copy versions if you can. See FFO for specific instructions.

[1] One computer file contains several linked spreadsheets.  Each spreadsheet is a different document.

[2] Develop the contents of this form using the mock application as a template.

[3] Submit this form only if instructed to do so after your application has been submitted. If this form is requested, it must be completed by the authorized representative identified on SF-424 and by any other person with significant project involvement.  Not required for employees of state government institutions.

[4] Complete this form only if you plan to subcontract at least $25,000 of your project.  Do not submit.  Keep it on file.   Any subcontractor for $25,000 or more will need to make the certification required by this form.
