Charles B. Wang Community Health Center Aims to Improve Diabetes Care and
Health Literacy of Patients with Diabetes
Charles B. Wang Community Health Center has joined
the New York City Health Department Diabetes Collaborative Care initiative. The Health
Center has developed a patient registry and is tracking clinical outcomes to
monitor and improve diabetes care. Currently, they have approximately
800 active patients with diabetes in the system. The health center is
developing new materials for health education and self-management support for
the Chinese community they serve.
With funding by Pfizer Health Literacy Foundation
to improve health literacy skills and self-management skills among patients
with diabetes, the Charles B. Wang Community Health Center has developed an
education handbook for low-literate Chinese people with diabetes that
incorporate National Diabetes Education Program (NDEP) messages. The health
center also conducts health
literacy trainings for physicians, nurses, health educators and others on how
to improve health communications and incorporate health literacy principles
in health materials. The health center offered patient education workshops in Chinatown
and Flushing that focused on physician-patient communication,
patient’s needs on diet control, and blood sugar monitoring, were followed
by improvements in A1C levels among these workshop participants. Health center
staff are planning an intervention manual for health educators and other health
care providers that includes a curriculum on delivering culturally-appropriate
care and self-management tools. They hope to have this ready for distribution
in May or June of 2004 for local partners and work group members.
