Diabetes Planned Visits

As the name implies, a planned visit is an encounter with the patient that the practice initiates to focus on aspects of care that typically are not delivered during an acute care visit. The provider's objective is to deliver evidence-based clinical management and patient self-management support at regularly scheduled intervals without the distractions inherent in the acute care visit.

Each of the four Academic Chronic Care Collaborative pilot sites adapted the basic concepts underlying the planned visit approach to meet the unique needs of the team and patient population. Sample overviews of each planned visit model include:

  1. University of Cincinnati Academic Health Center
  2. Summa Health System—Family Practice
  3. Oregon Health & Science University
  4. Vanderbilt University Medical Center

The University of Cincinnati Academic Health Center also conducted a group visit as an alternative to the traditional one-patient office visit. During a group visit, several patients attend meetings together to receive evidence-based clinical management and self-management support.

The Vanderbilt University Medical Center changed their delivery system to focus on foot exams and low-density lipoprotein (LDL) management.
