Figure H.2 is a vertical bar graph, representative of quality measures used in three different settings of care, and their improvement rates. The first and tallest vertical bar indicates measures of quality across 6 sampled hospitals improved at a median annual rate of 7.8 percent. A second and shorter vertical bar indicates measures of quality at 26 sampled ambulatory care settings improved at a median annual rate of 3.2 percent. The third and shortest vertical bar indicates measures of quality at 5 sampled nursing home/home health settings improved at a median annual rate of 1.0 percent.
