Evaluation
CDC will assist with providing objective program assessment. The aim of the
evaluation is to answer these overall questions:

Does the Healthy Steps model increase the number and percentage of EPSDT
visits and developmental screens performed at Swope?
-
Were the community outreach and education efforts successful in reaching
the intended populations?
-
Did the percentage increase of children with positive screens who were
referred to appropriate follow-up services?
-
How satisfied were the pediatric care providers with the Healthy Steps
model? Did the providers experience positive changes in their relationships
with patients and families?
-
What successes, barriers or challenges were there to program
implementation, and how were difficulties overcome?
This information will add to evaluations of Healthy Steps previously done
at other sites around the country. For more information, including
