NIOSH Respirator User Notices

Certificates of Approval Revoked for Wen Mask Industrial Company Model K4-795 N95 Filtering Facepiece Respirator (TC-84A-2531) and the KR-808 N95 Filtering Facepiece Respirator (TC-84A-2532)

The National Institute for Occupational Safety and Health (NIOSH) wishes to inform respirator users that the NIOSH certificates of approval for the Wen Mask Industrial Company model KR-795 N95 Filtering Facepiece Respirator (TC-84A-2531) and the KR-808 N95 Filtering Facepiece Respirator (TC-84A-2532) have been revoked effective July 3, 2002. The Institute has revoked these certificates of approval because of the failure of these Wen Mask respirators to meet the performance requirements of Title 42, Code of Federal Regulations, Part 84 (42 CFR Part 84), and the Institute's determination that Wen Mask is not ensuring effective quality control for these respirators as required by 42 CFR Part 84.

Revocation of approval means that the Wen Mask KR-795 N95 Filtering Facepiece Respirator (TC-84A-2531) and the KR-808 N95 Filtering Facepiece Respirator (TC-84A-2532), whether marketed by Wen Mask or under a private label, will no longer be listed as NIOSH-approved respirators. Revocation also means that respirators bearing these approval numbers may no longer be manufactured, assembled, sold, or distributed. NIOSH records indicate these respirators were approved to be marketed under the following labels:
