Before You Begin

In order to complete the OTA Membership Application please be prepared to:

  • Enter a business description (30 words or less) for your company's listing in The Organic Pages Online.
  • Select a dues category and level based upon your company's Gross Annual Organic Revenue in U.S. $.
  • Sign OTA's Code of Ethics.
  • Decide whether you want to participate in any of OTA's Member Forums.

There are two ways to apply:

  1. Click here for online form. Online form works best with Internet Explorer or Firefox with IE plug-in. Attention: MAC users may experience difficulty using Safari.
  2. If this form doesn't work on your computer, download a PDF version. Fill out and fax to (413) 774-6432 or e-mail to llutz@ota.com
