How to Apply for a Smithsonian Affiliation

Museums and cultural and educational organizations interested in affiliating with the Smithsonian Institution should submit a proposal to Smithsonian Affiliations. Please enclose a letter that describes the collections the museum would like to borrow from the Smithsonian. Also include programmatic initiatives the organization would like to explore.

All proposals should include the following:

	A profile summary sheet (enclosed)
	501(c)(3) documentation or status as a government-operated museum; copy of application acceptable
	Organization's mission statement
	Organizational chart
	Annual reports for the past two years (if available)
	Copies of Form 990, Return of Organization Exempt From Income Tax, for the previous two years (if available)
	Collections Management Policy
	A completed American Association of Museums Standard Facilities Report
	A brief statement of proposed projects or artifact loans
	Applicants should submit three (3) original copies of the proposal to: Smithsonian Affiliations Smithsonian Institution P.O. Box 37012, MRC 942 Washington, DC 20013-7012
