School and Childcare Assessment Report Forms


Every year we ask that you report immunization coverage in kindergarten and middle school. Please also report coverage biannually (during even years) in daycare.

We need this information in order to determine the overall immunization coverage estimates for your area (city, state, or territory) and for computing national estimates of coverage.  Please complete the childcare and school assessment reports by clicking on the links below. We will make the results available once we receive and analyze all of the data.

Hints and Help
 

  • Best results when viewed with Microsoft Internet Explorer
  • Your grantee ID is your password into the system
  • You may enter data several times before making a final submission. Just answer "No" to the "Is this your final submission of data for this survey" question.
  • If your survey results are final, and you do not intend to come back and edit data, answer "Yes" to the "Is this your final submission of data for this survey" question.
  • A copy of the data that you enter will go to the Assessment Branch and to the Program Operations Branch at the National immunization Program (NIP).
