Performance Management Appraisal Plan (PMAP) Form
The blank Performance Plan Form is currently available in several different formats to provide maximum flexibility in developing Performance Plans. We are working to modify and perfect the blank forms and will have additional versions available soon.
To use the forms:
- Open the blank form and save it to your computer
- Open a sample plan and save it to your computer (only applicable if you are copying the sample form)
- Open the saved versions of both forms to edit them
MS Word Form -- to check the boxes in this form you must double-click the box and select "Checked" under Default Value
