Certification Statements on Letterhead Certificates
Certification statements required by importing countries are sometimes too voluminous to include in the “Remarks” section of FSIS Form 9060-5, Meat and Poultry Export Certificate of Wholesomeness. In such cases, the certification statements are on letterhead certificates that are placed in the country requirements and can be easily downloaded for signature by FSIS inspection officials. These letterhead certificates should not be modified and additional statements should not be included on the preprinted form.

If importers demand that additional statements be included on the letterhead certificate or in the “Remarks” section of Form 9060-5, they should call the Office of International Affairs for further guidance at (202) 720-3473.
