INSTRUCTIONS FOR SUBMISSION OF RADIOGRAPHS FOR STUDY ON PROVENTRICULUS DISEASE

All radiographs submitted must be identified (either by radiographic marker or by file name if digital).

Digital Radiographs:

·         E-mail images in jpg format to: dennison@svm.vetmed.wisc.edu. Or e-mail for instructions to download DICOM data to a ftp site.

Conventional Radiographs:

·         Digitize using a digital camera. Make sure the only light in the room is coming from the light box behind the radiograph and block out excess light from around the radiograph. Images can then be e-mailed to: dennison@svm.vetmed.wisc.edu

·         Conventional radiographs may also be submitted via USPS:

Sophie Dennison, BVM&S
"PROVENTRICULUS STUDY"
Radiology Resident, Department of Radiology
School of Veterinary Medicine
University of Wisconsin
2015 Linden Drive
Madison, WI 53706

Please include your return address to expedite the return of radiographs to your clinic.

The diagnoses for each study submitted should be sent via a separate e-mail with a subject heading of "PROVENTRICULUS STUDY." Alternatively, the diagnoses may be submitted in a sealed envelope in the same package as any mailed radiographs. While this may be an inconvenience, it is necessary to prevent bias when the radiographs are read.

Additional Information Needed:

Clinical signs consistent with proventriculus disease?
Yes (explain)
No

Proventriculus disease confirmed?
Yes (explain)
No (Was another condition diagnosed?)

How was the diagnosis made? Fluoro, surgery, necropsy, culture, response to treatment?

Do you give me permission to acknowledge you/your clinic at the end of the manuscript?   Yes     No

Please note, only the proventriculus will be evaluated on radiographs submitted for this study. If you wish to have a radiographic study for an avian patient formally evaluated by a radiologist, it should be submitted to the radiography referral service as usual.

Thank you for submitting radiographs. Should you have any questions, please e-mail or call me at 608-262-5140. Without your help this study would not be possible. Even if you submit only one abnormal set, every radiograph helps.
