41st National Immunization Conference
Continuing Education Information for the 41st National Immunization Conference March 2007,
Course Number EV1053

Continuing education (CE) credits will be provided to participants who complete session evaluations and an overall conference evaluation using the online CE system.

Go to http://www.cdc.gov/phtnonline to access the online system and complete course evaluations.

Once you're logged in, go to the SEARCH AND REGISTER page, click on CDC Courses at the bottom right corner of the screen. The code to access CE credit is NIC2007.

CE expires on April 9, 2007. You must complete the online CE evaluations by this date.


CME Statement
This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the Centers for Disease Control and Prevention (CDC) and CDC Foundation, Centers for Medicaid and Medicare and the Task Force for Child Survival and Development . CDC is accredited by the ACCME to provide continuing medical education for physicians.
The Centers for Disease Control and Prevention designates this educational activity for a maximum of 16 category 1 credits toward the AMA Physician's Recognition Award. Each physician should claim only those credits that he/she actually spent in the activity.

CPE Statement
The Centers for Disease Control and Prevention is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program is a designated event for pharmacists to receive 1.6 Contact Hours in pharmacy education. The Universal Program Number is 387-999-07-009-L01.

CNE Statement
This activity for 16 contact hours is provided by the Centers for Disease Control and Prevention, which is accredited as a provider of continuing education in nursing by the American Nurses Credentialing Center's Commission on Accreditations.

CECH Statement
The Centers for Disease Control and Prevention is a designated provider of continuing education contact hours (CECH) in health education by the National Commission for Health Education Credentialing, Inc. This program is a designated event for the CHES to receive 16 Category I contact hours in health education, CDC provider number GA0082.

CEU Statement
The CDC has been reviewed and approved as an Authorized Provider by the International Association for Continuing Education and Training (IACET), 8405 Greensboro Drive, Suite 800, McLean, VA 22102. The CDC has awarded 1.6 of CEU's to participants who successfully complete this program.
