CPRS Templates Bank

Below are links to various pre-operative templates the National Anesthesia Service has received.  Review them and use or modify any that fit your local need.

WORD documents will show you the text of what the template contains.  Your Computer Applications Coordinator or CPRS expert should be able to use this information to create the note for you or help you modify it as you may need.

"TXML" documents can be downloaded for use by your Computer Applications Coordinator or your CPRS expert.  They are the "computer language" versions of templates and are ready to be imported into your CRPS.

 Anesthesia Pre-Op H&P Template:  This is the history and physical template that Seattle uses. It is being converted to a dialog template format, that will look and work better than the ASCII character template.  WORD  Seattle
 Anesthesia Pre-Op H&P Template:   This shows you a text version of what the "txml" document contains.  WORD  Seattle
 Anesthesia Pre-Op H&P Template  :  The "txml" file ready for import as is.  TXML   Seattle
 Clinical Warning & Rescinded Clinical Warning:   This note is used for the purpose, to act as a record flag when Anesthesia finds that a patient has a difficult airway intubation, or reacts to anesthetic drugs. The rescinded title is used when the condition no longer exists. There is no boilerplate text with this title.  WORD  Seattle
 Anesthesia Attending Note:  The Anesthesia attending for an operative case uses this progress note.  There is no template with this note.  WORD  Seattle
 Pre-Op Anesthetic Plan:  Pre-op assessment plan. Imbedded in the template are some of Seattle’s patient data objects. These probably need to be substituted for ones that your site uses.  WORD  Seattle
 Pre-Op Anesthetic Plan (alternative version):  This is another template that the Seattle residents can use prior to surgery.  WORD  Seattle
 Anesthesia Post-Op Note:  This note is used post-operatively. There is no boiler plated text.  WORD  Seattle
 Note for Attaching Scanned Anesthesia Record:  This note title is used when Seattle scans their anesthesia records into VISTA Imaging. There is boiler-plated text in the note, which states, “The attached scanned document can be viewed by opening VISTA Imaging.” Link here to web site information about how to implement the scanning process at your facility.  WORD  Seattle

 Anesthesia Attending Pre-Anesthesia Evaluation:  Directions from the facility = Attached is a pre-op template that we have been working with. It is in MS Word, and as a form.  Gets filled out, form unlocked and then copied to be pasted in a progress note in CPRS.  Working with our IRM to have it as a template in CPRS, and have CPRS automatically fill in fields, such as vital signs, lab work and x-ray.  Password to modify the form is  "jerry"< /FONT >

 Anesthesia History & Physical:  A "point and click" dialog template H&P in use at the Huntington, WV VAMC.  Embedded in the template are some of Huntington's patient data objects.  These may need to be substituted for ones your facility uses.  TXML  Huntington
