Plant Export Certificates and Forms

The Export Certificates and Application Forms have been separated into two separate categories: Downloadable and Viewable Forms.

The downloadable forms are Adobe PDFs that allow you to fill in fillable fields. You can fill out and print the form within your web browser. If you are using the free Adobe Reader software, you cannot save the form with the filled in data for use at a later time. You will need to use the purchased version of Adobe Acrobat in order to do this. The free reader software will, however, allow you to save the blank form for use at a later time.

The viewable forms are Adobe PDFs and for viewing purposes only. Viewing PDF files requires the Adobe Acrobat Reader. For your convenience, click here to download the Adobe Acrobat Reader software.

Downloadable Forms

PPQ Form 572, Application for Inspection and Certification of Domestic Plant and Plant Products for Export. Instructions for PPQ Form 572.

A form designed to provide the information needed to complete PPQ Forms 577 , 578, and 579, and to serve as a worksheet for the certifying official conducting the inspection. Although this form is required by regulation, in practice, alternative methods are used to obtain the necessary information for inspection and certification. When a PPQ Form 572, is used by an exporter as an application, the exporter should complete only the description of consignment section. Certifying officials should carefully check that extraneous information was not included on the form by the exporter.

PPQ Form 576, Attachment Sheet for PPQ Form 577 or PPQ Form 579. Instructions for PPQ Form 576.

The "Attachment Sheet for PPQ Form 577 or PPQ Form 579, PPQ Form 576 is used when the required information for phytosanitary certification does not fit on the original certificate. PPQ Form 576 can only include information that would normally be included on a phytosanitary certificate. The attachment sheet will be referenced on the corresponding phytosanitary certificate.

PPQ Form 577, View the Phytosanitary Certificate

The Phytosanitary Certificate, PPQ Form 577, is used to certify that the domestic plants or plant products have been inspected according to appropriate procedures, and they are considered to be free from quarantine pests, practically free from other injurious pests, and conform to the current phytosanitary regulations of the importing country.

PPQ Form 578, View an Export Certificate

The Export Certificate, Processed Plant Products, PPQ Form 578 is used to certify approved processed products. This export certificate affirms that, based on inspection or processing, the product is believed to be free from injurious plant pests. Moreover, issue a PPQ Form 578 only for articles specified in EXCERPT. Never issue this form for an article that warrants a PPQ Form 577.

PPQ Form 579, View a Phytosanitary Certificate for Reexport

The Phytosanitary Certificate for Reexport, PPQ Form 579, certifies that, based on the original foreign phytosanitary certificate and/or an additional inspection, the plants or plant products officially entered the United States, are considered to conform to the current phytosanitary regulations of the importing country, and have not been subjected to the risk of infestation of infection during storage in the United States.

Shipments of plants and plant products transiting the United States under Customs' bond are not eligible for reexport certification. Such shipments should be accompanied by a phytosanitary certificate from the country of origin if certification is required by the destination country.
