Number of Copies and Reminder of Due Date for CHGME Annual Report

Several hospitals have asked for clarifications regarding the format and the method of submission of the annual report.  The following provides you with guidance to address the question:

Number of Copies of Annual Report

The Annual Report submitted to HRSA on or before February 7, 2008 should include:

  • one (1) paper copy of completed Annual Report with the required signed certification
  • one (1) electronic copy of the completed Annual Report on CD. 
  • one (1) electronic copy of the Discharge Data by Zip Code on CD

Deadline for Annual Report

Your hospital’s completed, signed annual report package must be postmarked by February 7, 2008 and submitted to the:
