Successful Service Delivery Strategies
As
our nation grows and ages, we are working to ensure that the
needs of minority, low-income, and rural elders with Alzheimer's
disease are being addressed. Through
the Alzheimer's Demonstration Program, several models of care for various special
populations were developed. We encourage you to consider incorporating elements
from these models into your service delivery system.
