The
New Jersey Office of Child Care Operations strives
to continually improve the application process for parents who are
seeking child care services for their children - as well as for
service providers.
Click
on the links below for the most up-to-date application forms, along
with links to instructions, certification forms, co-pay schedules
and payment rates.
You
can download and print these forms, complete them by hand
and submit them according to the instructions.

Child
Care and Early Education Service Eligibility Application
This
form is required for all applicants and co-applicants seeking to
apply for or renew a
child care subsidy through a Child Care Resource and Referral Center
or contracted child care slot in a Center Based Contract (CBC) center;
it includes the new Parent/Applicant Certification form (see page
3, Section F).
(Providers
please note: A newly-signed Certification Form must also be attached
to all CARES redetermination applications, as part of the redetermination
process.)

Care and Early Service Eligibility Application Certification Form
This
form is part of the child care application and is used for all applicants
for subsidized child care services, including those at Center Based
Contract (CBC) child care centers.
The
Applicant
Instructions Page contains instructions on how to complete
the application forms.

Parents/Applicants: If you need help in completing your
application, please contact your local Child
Care Resource and Referral agency.

Providers: please continue to include your agency's name
and return address in the "Address Reply To" section on
page one of the application.
