CHAPS Mission
To build healthy communities and eliminate
disparities
by providing leadership in health promotion and chronic
disease prevention.

Community Health and Program Services (CHAPS) has a long and rich history
of supporting cross-cutting community health and health education functions
and houses programs that have expanded CDC’s ability to develop
community-based chronic disease prevention and health promotion strategies.
To accomplish CDC’s goals, CHAPS is taking steps to strengthen its capacity
to
- Serve as an expert center for community health promotion,
- Evaluate community-based health promotion strategies,
- Address the needs of local communities related to health promotion,
- Engage in discussions of integrated chronic disease prevention.
Exemplary CHAPS Programs such as Racial and Ethnic Approaches to
Community Health (REACH), the Preventive Health and Health Services (PHHS)
Block Grant, and YMCA’s Pioneering Healthier Communities have developed and
implemented interventions and prevention strategies that address chronic
illnesses at the local level through awareness campaigns, health education
classes, environmental changes, work site wellness interventions, and local
and state policy changes. In addition, these programs have documented
outcomes that demonstrate the effectiveness of local, community-based
strategies.
