  • Handelsman MM; Kemper MB; Kesson-Craig P; et al.; Use, Content, and Readability of Written Informed Consent Forms for Treatment. Professional Psychology: Research and Practice 1986; 17(6): 514-18. Seventy percent of the 104 psychologists responding to a survey indicated that they did not use written consent forms because they preferred oral agreements. Those psychologists who did use consent forms disclosed payment information but not risks and treatment alternatives. The mean readability of the consent forms returned by sixteen respondents was grade level 12.5 according to the Fry test. [Source: Sugarman, J]
