Promise
to the State: This document outlines the steps
that will be taken to make the provision of medical homes
for children with special health care needs a reality
by the year 2010.
Practice-Based
Quality Improvement Activities: This document
notes the quality improvement plan to assist practices
in increasing their medical homeness (this can include
a learning collaborative strategy or an EPIC model). It
includes a brief description of the strategy and tools
being utilized along with contact information.

Through analysis of the State Mentor Teams, several common
activities occurred with each one that contributed to their
success. Among these are:
- Establish regular meetings with the Medical
Home Team to plan and strategize how to integrate and
implement related activities. Ask the medical
home contact listed above when the team currently meets—can
you join their discussion? OR can a member of the medical
home team attend one of your naturally occurring meetings
regarding related activities to give an update on state
medical home activities?
- Expanding the core team to include all
of the key stakeholders in the state. Mentor teams
discovered that "buy-in" to the state plan was
necessary in all state organizations and agencies at all
levels.
- Take advantage of existing groups and/or committees
that have shared goals and an invested interest in your
activities to help in the continued development of integrating
your activities with the medical home. Early Intervention,
AAP State Chapter, AAFP State Chapter, Title V-CSHCN,
Family Advocacy Groups.
- Establish a mechanism of outreach whether
through a hands-on approach that assists physicians in
improving their practice, or developing a supportive network
of physicians across the state. Mentoring teams
to visit practices and promote your related activity/
Medical Home integration, email “bulletin board”
for practitioners and families to communicate, organize
a meeting for practitioners involved in your related activities
to discuss lessons learned, etc. at your AAP state chapter’s
annual meeting.
- Provide education or training on your
activity and medical homes to diverse audiences and through
multiple media. One-day training programs, brochures,
newsletters, fact sheets to guide practitioners through
the referral process and on-going care management, etc.
As this map shows, there
are a multitude of activities and methods you can use to
engage individuals in your state to make the commitment.
