4.4. Links to NHIS Tobacco Questionnaires on Web

This page contains the names of the National Health Interview Survey (NHIS) questionnaire files that contain tobacco questions, along with their links to the NHIS website.  Note that these are the PDF files for the questionnaires only, not for the data files, nor for the codebooks that detail the locations of the tobacco related variables in the data files.

From 1997 on, the Sample Adult module of the NHIS core questionnaire contained questions on cigarette smoking every year.  Additional tobacco use questions were asked in supplements fielded in 1998, 2000, and 2005.

Before 1997,  tobacco questions were not part of the core questionnaire but were placed in supplements that were not asked every year.  The questionnaires and supplements were generally printed in Appendix III of the Current Estimates report for each year.

In 1978, 1979, and 1980, the smoking supplement questionnaires were printed on separate pages that were not included in the published version of Current Estimates for those years.  We provide a link to a report with a copy of the 1980 NHIS supplement in Appendix IV.
