Title: Disparities in Quality of Healthcare Affect Utilization

Summary: This Mentored Clinical Scientist Development Award will attempt to determine the impact of disparities in the perception of primary care quality on health care utilization patterns for a nationally representative population (from the Medical Expenditure Panel Survey), and a low-income, high-risk pediatric population (the Wisconsin Medicaid administrative database). The researcher will also define the parental perspective on non-urgent use of the emergency department (ED), and determine the effect of the parent/provider relationship on non-urgent ED utilization using focused interviews with parents who brought their children to the ED with non-urgent conditions. Finally, the project will also define the primary care provider perspective on non-urgent utilization of the ED, and determine provider responses to the views expressed by parents from their practice. Low-income, inner-city and minority (Hispanic or Latino, American Indian/Alaska Native, Asian, Native Hawaiian or other Pacific Islander, and African American) children and parents will be included in the study.

Summary: This Independent Scientist Award is intended to enable the investigator to reach his goal of becoming a leading child health services researcher who focuses on children who are minority, poor, and underserved. The aims of the research project funded by this grant are to: (1) use focus groups to identify the reasons why parents are unable to obtain health insurance for their uninsured children, with an emphasis on Latino families; and (2) conduct a randomized trial to evaluate whether specially-trained community-based case managers are more effective than traditional methods in reaching out to and enrolling uninsured children in public insurance programs.
