VA Service & Distribution Center - VA Forms & Publications
VA Forms and Publications maintains approximately 1,165 line items utilized by Dept of VA through-out the world required for patient care and veteran support. We also support other government agencies, such as DOD, State Governments, Embassys and other veteran support agencies.

Maintains inventory control, replenishment analysis, receiving, packing and shipping of all VA forms and Publications.
- Creates and updates an electronic catalog
, which is updated monthly, with current additions, changes and deletions.
- Performs Initial Distributions as required for new forms/publications as specified by the Department responsible for the original item.
- Access VA forms online.
For Non-VA requests for forms/publications; contact your local VA facility.

- VAMC’s may place their order through its station’s PCO (Forms/Publication Supply officer).
- Forms and publications may be ordered using Forms and Publications (FP) Orders System through the Austin Automation Center.
- FP Orders system users can access a complete listing of stocked forms and publications.
- FP Orders System requests are typically shipped within four workdays of receipt at the SDC.
- To establish an FP Orders System account, contact your local Automated Customer Registration System representative (ACRS) or Information Security Officer (ISO). The Austin Automation Help Desk can be reached at (512) 326-6780 if you are unsure who that is at your local station.
- Veterans Outreach Centers and VA Cemeteries are to order through their Servicing Station.
- Printed catalogs containing a complete listing of all VA forms and publications are available free of charge by calling the SDC. For your convenience, we have developed downloadable/printable versions of the VA Forms and Publications Catalog .
