Share your Stories, Share your Comments!

We want to learn from you!
Please share your experiences with us. Your input will help us improve the School Health Index (SHI) and encourage additional schools to improve the health of their students, staff, and families. To help you get started, here are a few ideas:

  • Describe ways in which you have promoted the SHI.
  • Tell us about your implementation process (e.g., who was involved, how long it took).
  • Describe how you have adapted the SHI (e.g., added health topics, created online version).
  • Describe changes made in your school because of the SHI.
  • Share challenges or lessons learned from the SHI process.
  • Tell us how you used the results from the SHI to develop partnerships or gain community support.
  • Give suggestions on how the SHI could be enhanced in the future and/or what CDC can do to help schools implement the SHI.

Share your story, comments, or questions using the form below.
If you prefer, you may e-mail us directly at cdcinfo@cdc.gov.
