Violation:
THE DISTILLATE TRANSFER LINE, USED TO TRANSFER DISTILLATE FROM THE VESSEL TO SHORE WAS PAINTED BLUE SIMILAR TO THE POTABLE WATER FILLING LINE. THE DISTILLATE TRANSFER LINE IN THE AFT PORTSIDE BUNKER STATION WAS PAINTED A BLUE AND WHITE COLOR AND STRIPED BLUE AND YELLOW DIFFERENTIATING IT FROM THE POTABLE WATER LINES.
THE HOSE FITTING WAS DIFFERENT FROM THE POTABLE WATER FILLING LINE CONNECTIONS DECREASING THE POTENTIAL FOR CONNECTION MISTAKES.

HOSES WITH SHUT-OFF VALVES WERE CONNECTED TO THE HOSE CONNECTIONS ON THE HANDWASHING STATIONS. ATMOSPHERIC VACUUM BREAKERS WERE PROVIDED AS THE BACKFLOW PREVENTERS. |
