Item No.:

Site:
Potable Water-Potable Water- Beauty Salon
Violation:
The reduced pressure assembly backflow preventer in the deckhead of the beauty salon had a funnel for the vent installed so that it covered the vent and did not allow a visual inspection for leakage.
Recommendation:
Physically separate the receiving funnel for the reduced pressure backflow preventer vent to a distance which will allow a clear view of the vent and any water leaking from it.
Site:
Potable Water-Potable Water- Tank Cleaning and Disinfection
Violation:
Potable water tank #1 port was cleaned and disinfected in October 2006. In the electronic record there was no documentation of the flushing or other dechlorination after disinfection and prior to or as part of the filling to ensure a free halogen residual below 5 ppm was present in the tank. This same violation was noted when the ship was last inspected by the VSP in March 2006.
Recommendation:
Ensure that documentation of any inspection, cleaning, disinfection (including concentration and contact time of disinfectant), and flushing is maintained for 12 months and is available to the VSP for review during inspections. 5.3.4.1.6 The disinfected parts of the system shall be flushed with potable water or otherwise de-chlorinated until the free residual halogen is less than or equal to 5.00 mg/L (ppm).
*Inspections scores of 85 or lower are NOT satisfactory
