Site: Potable Water-DECK 4 CHEMICAL STORAGE PANTRY #4531
Violation: THE BACKFLOW PREVENTER INSTALLED AT THE POTABLE WATER SUPPLY LINE CONNECTION TO THE HANDWASH STATION WAS LEAKING CONTINUOUSLY. THIS LINE WAS ALSO CONNECTED TO THE VARIOUS DISPENSERS FOR CLEANING CHEMICALS ATTACHED TO THE BULKHEAD BESIDE. THE PREVENTER WAS REMOVED DURING THE INSPECTION.
Recommendation:Repair or replace the leaking backflow preventer.
Site: Potable Water-COMPREHENSIVE CROSS-CONNECTION CONTROL PROGRAM
Violation: THE ELECTRONIC RECORD OF BACKFLOW PREVENTION DEVICES REQUIRING INSPECTION ONLY, WAS NOT SPECIFIC ABOUT WHAT THE DEVICE WAS CONNECTED TO. THE RECORD SIMPLY DESCRIBED NON-TESTABLE BACKFLOW PREVENTERS ON DECK ...., OR SOME NON-TESTABLE PREVENTERS FOUND LEAKING AND REPLACED ON DECK ....
Recommendation:Modify the backflow prevention device reocrds to indicate the device to which the backflow preventer is connected.
Site: Potable Water-ENGINE ROOM - SEA CHEST PORTSIDE FORWARD
Violation: A DECK WASHING TAP SUPPLIED FROM POTABLE WATER HAD A HOSE SECTION CONNECTED WITHOUT ANY BACKFLOW PREVENTER INSTALLED. THE HOSE WAS RESTING IN CONTACT WITH A LARGE BIOGARD CHEMICAL CONTAINER OVER HALF-FULL WITH THE BIOGARD CHEMICAL. THE HOSE WAS DETACHED FROM THE WATER LINE IMMEDIATELY.
Recommendation:Install the appropriate backflow prevention device for this application.
Site: Potable Water-CANOPY EXHAUST HOOD WASH
Violation: THE CONBRACO DOUBLE CHECK ASSEMBLY BACKFLOW PREVENTERS INSTALLED IN THE HOOD CLEANING TECHNICAL CABINETS OF ALL GALLEYS WERE NOT MANUFACTURED WITH INTERMEDIATE ATMOSPHERIC VENTS. THESE APPEARED TO BE FROM THE ORIGINAL CONSTRUCTION OF THE SHIP.
Recommendation:Replace these backflow prevention devices with double check valve assemblies with imtermediate atmospheric vents.
Site: Potable Water-GARBAGE ROOM - DECK 4
Violation: THE DECK WASH TAP IN THIS SPACE HAD A SPRAY RINSE NOZZLE CONNECTED TO THE HOSE, SUBJECTING THE HOSE BIBB BACKFLOW PREVENTER INSTALLED TO CONTINUOUS PRESSURE. THE BACKFLOW PREVENTER INSTALLED WAS A NON-CONTINUOUS PRESSURE MODEL. THE BACKFLOW PREVENTER WAS REPLACED DURING THE INSPECTION.
Recommendation:Install a backflow prevention device in this location that is designed for continuous pressure applications.
Site: Potable Water-INCINERATOR ROOM - DECK 3
Violation: THE POTABLE WATER SUPPLIED DECK WASH TAP AT WATER PRESS #5 HAD A LENGTH OF HOSE ATTACHED WITH A SPRAY RINSE NOZZLE ON THE END. THE BACKFLOW PREVENTER INSTALLED ON THE HOSE BIBB WAS DESIGNED ONLY FOR NON-CONTINUOUS PRESSURE APPLICATION.
Recommendation:Install a backflow prevention device in this location that is designed for continuous pressure applications.
