Service Delivery Models
You are not alone in your effort to address the needs of persons with Alzheimer's disease and their families. Many states and organizations have developed successful models of care for Alzheimer's clients through the Alzheimer's Demonstration Program. In the Successful Service Delivery Models section, you'll find detailed information about unique and successful service delivery models in a number of states across the country. Many of the programs focused on serving minority, low-income and rural Alzheimer's families. The elements of these models can be successfully replicated in most communities, so before you reinvent the wheel, check out our info.

In addition, through a national evaluation of various Alzheimer's Demonstration Programs, a number of elements were identified as being necessary components and activities for the success of any community-based service development effort. These strategies are presented in the Program Development Strategies page of this section.
