Successful Service Delivery Strategies

Since 1992, the Alzheimer's Demonstration Program has focused on developing unique community-based methods of serving persons with Alzheimer's disease and their families. Specifically focusing on serving hard-to-reach groups such as ethnic minorities, low income and rural families, Alzheimer's Demonstration grantees have developed many replicable models for working with traditionally underserved populations.

Through a national evaluation process, the most replicable components of various Alzheimer's Demonstration projects were identified and are presented here. Each link provides a description of the model and contact information for the program. You should feel free to contact the project director for more details and for technical assistance in replication. We strongly encourage you to consider incorporating elements from the successful strategies below into your home and community based care system.
