Revised Adult HIV Treatment Recommendations

On October 10, 2006, the DHHS Panel on Antiretroviral Guidelines for Adults and Adolescents released a new revision of the Guidelines for the Use of Antiretroviral Agents in HIV-1-Infected Adults and Adolescents.

 

The Panel revised its recommendations for preferred and alternative antiretroviral components when initiating antiretroviral therapy in treatment-naïve patients.  These recommendations are based on reported results from several randomized trials and on safety data that have emerged since the last revision. Specific recommendations are deatailed in Table 6a.

 

Several other options are considered acceptable as initial components but often inferior to the preferred or alternative components. These options include nelfinavir, ritonavir-boosted saquinavir, stavudine + lamivudine, and a triple-NRTI regimen containing abacavir + zidovudine + lamivudine. Rationales for these options are detailed in a new Table 6b as well.

 

Existing tables are updated with recent trial results and with drug information about new drugs darunavir and Atripla. Black box warning data for patients taking tipranavir has been added, and a new table, Table 30 provides expanded access information about two investigational agents, TMC125 (etravirine) and MK-0518.
