Contact Information Form

Standard Form 1199A

Supplemental Form

Grant recipients that have an existing Smartlink Payee Account established with the U.S. Fish and Wildlife Service must contact the Division of Bird Habitat Conservation at DBHCSmartlink@fws.gov and provide the following:

  1. Project title
  2. Organization name
  3. DUNS number
  4. Smartlink Payee Account Number
  5. Confirmation that you want these new funds deposited in this Smartlink account
  6. Confirmation that the name of the person requesting payments has not chanfed
  7. Confirmation that the banking information on file has not changed.

Instructions for Setting up a SmartLink Account

Only if the Division of Bird Habitat Conservation has instructed you to do so, should you setup a SmartLink account.

SmartLink is an easy-to-use, electronic draw down payment system. Payment funds requested and approved through SmartLink are delivered the next business day. It is important to note that payments to banks located outside of the USA may take several days longer.

Step 1. Provide us with your Dun and Bradstreet Data Universal Numbering System (DUNS) number. The DUNS number is a unique nine-character identification number provided by the commercial company of Dun & Bradstreet. It is a unique identifier for single business entities and is assigned for each physical location in your organization. Visit our Dun and Bradstreet contact page for details on obtaining a DUNS number.

Step 2. Complete the CONTACT INFORMATION FORM-SMARTLINK

Step 3. Complete the DIRECT DEPOSIT SIGN-UP FORM (Standard Form 1199A-Front) DIRECT DEPOSIT SIGN-UP FORM (Standard Form 1199A-Back)

The person(s) with the authority to access your bank account must complete the Direct Deposit Sign-up Form.

INSTRUCTIONS FOR COMPLETING THE DIRECT DEPOSIT SIGN-UP FORM

Step 4. Complete the SUPPLEMENTAL FORM-DIRECT DEPOSIT only if your destination bank is located outside of the USA (this will be the case for most foreign grantees). It is important to note that you may incur administrative costs and/or transaction fees for payments wired from the SMARTLINK system to banks located outside of the USA. Please contact your local bank to learn more.

INSTRUCTIONS FOR COMPLETING THE SUPPLEMENTAL FORM-DIRECT DEPOSIT

Step 5. Mail the original forms to the address below. You will be unable to request payment(s) until all required forms have been received and processed by the Division of Bird Habitat Conservation. It takes approximately 20 business days from receipt of correctly completed forms to establish a SMARTLINK account. You will be notified by email when the account is established and when funds are deposited in the Smartlink account.

Since these forms are related to financial information, they may not contain any evidence of changes or corrections, including whiteout or strikethrough. Forms with alterations or corrections will be returned with a request to re-submit.

Changes and Updates

Contact us immediately should any of the information on the above forms change. You will be provided instructions on how best to address the necessary revisions. Generally, you will need to submit new forms.
