Card Acquiring ServiceNew ApplicationIn order to participate in the Card Acquiring Service, agencies must complete a Card Acquiring Service New Application and submit to FMS for approval. An application is required for:
*New Accounts are also required to complete an Agency Participation Agreement Please return the completed Card Acquiring Service New Application form to Barbara Harper: e-mail: barbara.harper@fms.treas.gov or fax: (202) 874-6575. For questions about this form, please contact Barbara Harper at (202) 874-6936 or Dena Corson at (202) 874-0807. Turnaround time for FMS application approval is approximately two weeks. Once approved by FMS, your application will be sent to Fifth Third Bank for processing. Fifth Third will set up the merchant account(s) and contact your agency with any additional questions. Your new account information will then be provided to you. |
