Virginia Dept. for the Aging Center for Rural & Geographically Isolated Populations
Will focus on vulnerable and hard-to-reach and geographically isolated populations. These individuals, by virtue of where hey live, have not been sufficiently educated in order to empower them to monitor their health care. They reside in areas that make it difficult to sufficiently provide educational and informational opportunities.

Commonwealth of Puerto Rico Dept. for the Aging Center for Rural and Non-English Speaking Populations
Focus on education and outreach to low literacy, non-English speaking populations who are also vulnerable because they live in hard-to-reach, rural and geographically isolated areas. Because these populations do not use English as their first language they are at high risk for fraud and abuse. The language barrier, in combination with their geographic locations, requires unique outreach and educational strategies in order to effectively ensure their health care.
