Violation:
In the production halogenation logs, there are columns for entering the halogen levels every 4 hours. The column headings are pre-entered with the times - every 4 hours beginning at 0200. The time that production is started and stopped is also entered in the logs. However, if the production start time is in between one of the pre-entered headings, the first reading is entered into the next column and the actual time of the sample is not recorded. On one occasion, the production start time was 0001 and the halogen reading was recorded in the 0200 column. Staff stated that the halogen is sampled and confirmed to be above 2 ppm at the beginning of the production, but this was not indicated in the log.

Recommendation:
Ensure that actual time of the first halogen reading of 2.0 ppm or higher is recorded.
