    Cruise Ship's Corrective Action Statement

Item No.: 01
Site: Medical-GI Report (24-hour Report)
Violation: For the voyage ending 10 Aug 2008, one crew case met the case definition, but was not included on the 24-hour report.
Recommendation: The gastrointestinal illness report shall contain: (1) The name of the vessel; (2) The ports of embarkation and disembarkation; (3) The dates of embarkation and disembarkation; (4) The total numbers of reportable cases of gastrointestinal illness among passengers and crew members, including those who have been disembarked or removed because of illness, even if the number is 0; and (5) The total number of passengers and crew members on the cruise.

Site: Potable Water-Cross-Connection Control Listing
Violation: The Ecolab connections for various chemical and or soap dispensing throughout the ship was listed in the cross-connection control logs, but there was no backflow preventer type provided with each type of dispenser. At least two different preventer types were found (air-gap and atmospheric vacuum breaker), but there may be more.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Buffet-Windjammer
Violation: The time as a public health plan control plan did not list the service line opening and closing times for embarkation day. Per staff buffet service begins at 0600 and ends at 0900 on embarkation day.
Recommendation: Provide the service times on the written time as a public health plan.

Site: Galley-Breakfast Area
Violation: There was an unlabeled working container of liquid eggs and an unlabeled working container of cooking oil being used on the flat grill.
Recommendation: Ensure that working containers holding food or food ingredients that are removed from their original packages for use on the vessel, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar are identified with the common name of the food.

Site: Other-Portofinos/Chops
Violation: The iron plates in the bottom of the ovens were corroded.
Recommendation: Ensure that materials used in the construction of multiuse utensils and food-contact surfaces of equipment are: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Site: Galley-Pastry
Violation: Two of the large table-top cutting boards were scratched and scored, making cleaning difficult.
Recommendation: Ensure that surfaces such as cutting blocks and boards that are subject to scratching and scoring are resurfaced if they no longer can be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

Violation: Excess and rough sealant was found in the technical compartment of the two-compartment sink, making cleaning difficult.
Recommendation: Remove the excess sealant

Violation: There was glue residue on the exterior front surfaces of the ovens. There were also four missing equipment data plate screws, creating void spaces.
Recommendation: Remove the glue residue and replace the missing screws on the oven exteriors.

Violation: There were loose handles/knobs and chipped exterior wood surfaces on the storage cabinet below the cappuccino machine.
Recommendation: Ensure that nonfood-contact surfaces of equipment and utensils are designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.

Site: Buffet-Jade Service Line Port Beverage Station
Violation: The ice machine cuber cover had a light mold growth on the interior and exterior surfaces. This was cleaned at the time of the inspection.
Recommendation: Ensure that food only contacts surfaces of equipment and utensils that are cleaned and sanitized.

Site: Buffet-Jade Service Line Port Beverage Station
Violation: The ice machine cuber cover had a light mold growth on the interior and exterior surfaces. This was cleaned at the time of the inspection.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.

Violation: In a previously cleaned area, two of the large table-top cutting boards were soiled.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The bottom surfaces of two induction plates were found soiled and stored inside an undercounter storage cabinet.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The interior of a storage cabinet was found with a white, chalky residue.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The technical space of the upright reach-in refrigerator was soiled.

Violation: There were two large plastic bins stored behind a combination oven. This was corrected during the inspection.
Recommendation: Ensure that cleaned equipment and utensils, laundered linens, and single-service and single-use articles are stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 150 mm (6 inches) above the deck.

Violation: The deck under the mat was rough and had small amounts of debris. A loose profile strip was found on the bulkhead at the pantry entrance.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: The deck under the mat was rough and had small amounts of debris. A loose profile strip was found on the bulkhead at the pantry entrance.

Violation: One live drain fly was found in this area.
Recommendation: Ensure that the presence of insects, rodents, and other pests is effectively controlled to minimize their presence in the food storage, preparation, service areas, warewashing, and utensil storage areas aboard a vessel.
