Item No.: 02
Site: Medical-
Violation: There were no gastrointestinal illness questionnaires available to provide to passengers or crew members with gastrointestinal illness.
Recommendation: Ensure that questionnaires are available to provide to passengers and crew members who report to medical with gastrointestinal illness.

Site: Potable Water-Sprinkler Shore Connection
Violation: The reduced pressure principle backflow prevention device for the sprinkler shore connection was installed vertically. It was not clear that this device is approved for vertical installation.
Recommendation: Ensure that backflow prevention devices are installed according to the manufacturer's guidelines.

Site: Potable Water-Laundry Room
Violation: The potable water pipes in the laundry room were not painted or striped blue.
Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Site: Galley-Officer's Pantry
Violation: There was a non-continuous pressure-type backflow prevention device on the potable water line to the coffee machine.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Potable Water-
Violation: The potable water hose connections in the engine room were not included in the vessel's comprehensive cross-connection control program list of backflow prevention devices.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Galley-Cooling Logs
Violation: The hot items that are cooled for some cold salads that the staff stated were served onboard such as chicken salad and pasta salad were not indicated on the cooling logs.
Recommendation: Ensure that logs documenting cooked, potentially hazardous food cooling temperatures and times from the starting points designated in 7.3.5.2.1 thru the control points at 2 and 6 hours are maintained onboard the vessel for a period of 30 days form the date the food was placed in the cooling process. Ensure that logs documenting cooling of potentially hazardous foods prepared from ingredients at ambient temperatures, with the start time to the time when 5°C (41 °F) is reached are also maintained for a period of 30 days, beginning with the day of preparation.

Site: Galley-Officer's Pantry
Violation: The upper front left corner of the ice bin had two small holes and the sealant was damaged.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: There was a domestic-type microwave located in the officer's mess.
Recommendation: Ensure that food-contact surfaces of food equipment complies with American National Standards Institute (ANSI) or other internationally accredited food-equipment sanitation standards for materials, design, and construction.

Site: Other-Car Deck Ice Machines
Violation: The left front juncture of the upper and lower compartment of the left ice machine was damaged and had an opening between the two sections. New ice bin sections for the ice machines were onboard to replace the 2 ice machine bins in this location.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Site: Food Service General-Ice Machines
Violation: Sikaflex-291 was used as the sealant inside ice machines. The tube of this sealant did not indicate that it was approved for food contact surfaces.
Recommendation: Remove the sealant. If sealant is needed in this location, ensure it is approved for use in food contact areas.

Site: Food Service General-
Violation: Non-food contact surfaces of equipment were difficult to clean due to gaps, seams, and other features.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Bar-Princess Bar
Violation: The upper corners of the gaskets for the undercounter refrigeration units were torn.
Recommendation: Replace the gaskets.

Site: Galley-Dishwash
Violation: The main dishwash machine was out of order. The glasswash unit was operational and the vessel was using plastic disposable plates at the buffet.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Violation: The white pipes to the right of the ice cuber in the upper compartment of the ice machines were soiled on the underside of the pipes.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The gaskets to the upright refrigeration units were lightly soiled along the edge near the latch.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The track for the gate of the dumbwaiter was heavily soiled with food debris. The lower area around the opening inside the door was also soiled.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Food Service General-
Violation: Decks were difficult to clean due to cracked tiles, rough surfaces, piping on the deck, and missing grout. Bulkheads and deckheads were difficult to clean due to loose profile strips and piping attached to the surfaces.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: The faucet at the utility sink was dripping.
Recommendation: Ensure that a plumbing system in a food area is maintained in good repair.

Violation: The waste water from the glass wash machine backed up into the handwash sink.
Recommendation: Ensure that a plumbing system in a food area is maintained in good repair.

Site: Galley-Dishwash
Violation: There was a small roach on the bulkhead on the left of the entrance, across from the dishwash machine.
Recommendation: Ensure that the presence of insects, rodents, and other pests is effectively controlled to minimize their presence in the food storage, preparation, service areas, warewashing, and utensil storage areas aboard a vessel.

Site: Integrated Pest Management-
Violation: There was no requirement in the vessel's pest management program guidelines for night-time active monitoring for pests. There was also no documentation of these night time inspections, but staff stated they were completed.
Recommendation: Ensure that the Integrated Pest Management Plan sets a schedule for periodic monitoring inspections including some at night.

Site: Housekeeping-Outbreak Prevention and Response
Violation: There was no written outbreak prevention and response plan onboard.
Recommendation: Ensure that each vessel has a written Outbreak Prevention and Response Plan (OPRP) which details the standard procedures and policies to specifically address gastrointestinal illness onboard. The plan shall include the duties and responsibilities of each department and their staff to include steps in outbreak management and control as well as the trigger for each step.

Site: Ventilation-
Violation: The condensation collection area of unit T9 was soiled with a black slimy material. This unit was located on the Florida Deck.
Recommendation: Ensure that air handling units are kept clean.
