    Cruise Ship's Corrective Action Statement

Item No.: 02
Site: Medical-
Violation: Shipping labels were not available for the shipping of specimens.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Site: Potable Water-
Violation: The potable water hoses were not stored capped or coupled together.
Recommendation: Ensure that potable water hoses are stowed rolled tight with the ends capped, on reels, or racks or with ends coupled together in potable water hose lockers.

Site: Potable Water-
Violation: The log for backflow protection devices in the passenger accommodations only showed the devices connected to the potable water lines for the showers. Additionally, the log incorrectly identified these devices as S8 devices. The log indicated that the cabin stewards were to inspect these devices for leaks and report to engineering. Given this it was not clear if the devices connected to the potable water lines supplying the toilets were inspected. These devices were located in cabinets in the passenger accommodations hallways.
Recommendation: Ensure that all backflow protection devices in passenger accomodations are on an inspection schedule.

Site: Potable Water-
Violation: Air gaps were not provided on the drain lines connected to the vents on the reduced pressure backflow protection devices on the evaporator line to the technical tanks near the mineralizer. Additionally, an air gap was not provided at the discharge line for the mineralizer backwash lines.
Recommendation: Provide air gaps at these locations.

Site: Whirlpool/Spa-
Violation: It did not appear that there was an automatic shut-off for the water heater to prevent temperatures in excess of 40°F.
Recommendation: Ensure that a temperature control mechanism to prevent the temperature from exceeding 40°C (104°F) is provided on whirlpool spas.

Site: Swimming Pool-
Violation: While the fecal accident log indicated the chlorine residual used during disinfection following fecal accidents there was no indication of the contact time.
Recommendation: Ensure that the fecal accident procedure provides a contact time to be followed during disinfection.

Site: Buffet-
Violation: Side sneeze guards were not provided on the crew and staff mess buffet lines. Staff stated that food was placed near the edge of the counter.
Recommendation: Provide side sneeze shields on the buffet lines.

Site: Provisions-
Violation: Two containers of frozen chicken were stored uncovered on the top rack of the deck 4 poultry walk-in refrigerator.
Recommendation: Provide covers for foods stored on the top shelf of this unit.

Site: Galley-
Violation: The surfaces of the grooved grills on deck 4, 5, and 6 were slightly pitted, making cleaning difficult. Per the staff, these grills were replaced three months ago.
Recommendation: Ensure that materials used in the construction of multiuse utensils and food-contact surfaces of equipment are: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Site: Galley-Deck 6
Violation: A few previously cleaned plates were slightly soiled with food debris and stored as clean.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.

Site: Galley-Deck 5 Starboard Forward
Violation: The temperature of the handwash station was measured at 146°F. The temperature of this unit was not adjustable by the user. This was corrected during the inspection.
Recommendation: Ensure that handwashing sinks are equipped to provide water at a temperature of at least 43°C (110°F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, where the user cannot make temperature adjustments, ensure that the temperature provided to the user does not exceed 52°C (125°F).

Site: Galley-Deck 5 Starboard Aft
Violation: Slight corrosion was noted at the base of the bulkhead near the mooring deck entrance.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Site: Children Area-
Violation: The exclusion policy for ill children did not include a provision requiring the medical staff to clear the ill child to return to the center. Per the staff this would be done but it was not in writing. This was corrected during the inspection.
Recommendation: Ensure that children with infectious illness are not allowed in the child-activity center without permission of the vessel's medical staff.

Site: Children Area-
Violation: The chlorine disinfectant for the diaper changing table was mixed with an all purpose cleanser and placed in a spray bottle. A test strip showed no visible color change. In addition, there was no indication on the bottle that this was a disinfectant or what exactly was in the bottle.
Recommendation: Do not mix disinfectants with cleansers. Properly label disinfectants used in the center.
*Inspections scores of 85 or lower are NOT satisfactory
