Certificates of Birth Resulting in Stillbirth

The State Bureau of Vital Statistics and Registration maintains records to prepare Certificates of Birth Resulting in Stillbirth from 1969 to the present.

The parent(s) can request a certified copy of a Certificate of Birth Resulting in Stillbirth by completing the REG-68 form [pdf 19k] [Word 35k] and forwarding the completed form, Acceptable Forms of Identification and appropriate Fees for a certified copy of a vital record to a licensed health care professional for signature.

The licensed health care professional must submit the form and above items to:
