The parent(s) can request a certified copy of a Certificate of Birth
Resulting in Stillbirth by completing the REG-68 form [pdf 19k]

the completed form, Acceptable
Forms of Identification and appropriate
Fees for a certified copy of a vital record to a licensed
health care professional for signature.
The licensed health care
professional must submit the form and above items to:
