Health Card Instructions

 

New & Renewal Health Cards
 
  1. Health Cards are available at Ravenholt Public Health Center, East Las Vegas Public Health Center, Henderson Public Health Center and the Cambridge Community Outreach Center between the hours of 8 a.m. and 4 p.m., Monday through Friday. Visit the Locations page for addresses and maps.

    Visit the Mesquite and Laughlin webpages for information about health cards in those locations.

  2. The fees are as follows:
    1. Food handler, massage therapy & health club attendant cards - $50; valid for 3 years.
    2. Child care, reflexology, aesthetician, barber & body art cards - $40; valid for 2 years.
    3. Adult group care cards - $45; valid for 2 years.

      An additional $15 late fee will be applied to card renewals that have expired.


  3. Bring two forms of valid identification, one of which must contain a photo (see valid i.d. list).

  4. When you arrive, you will fill out an application form, or you may print a PDF form in English PDF (26 KB/1 page) or Spanish PDF (26 KB/1 page), fill it out, and bring it with you accompanied with two (2) forms of valid identification (see valid i.d. list).

  5. You will receive a Hepatitis A vaccination for a Food Handler card.
    • Please bring your immunization record with you.
    • Clients under the age of 18 must have a parent or legal guardian present or a letter signed by the parent or guardian, giving us permission to immunize the minor.

  6. If you need a childcare card, adult group care card, or other type of health card, you will receive a Hepatitis A vaccination and Tuberculosis screening.
    • Please bring your immunization record with you. If you have a history of a positive TB test, please bring the appropriate documentation.
    • Clients under the age of 18 must have a parent or legal guardian present or have a letter signed by the parent or guardian, giving us permission to immunize the minor and/or screen for TB.
    • TB skin tests are not administered on Thursdays or on days when they cannot be read in 48 to 72 hours.

  7. If you need a tattoo, permanent makeup, or body piercing health card, please visit the Body Art Health Card Requirements web page for detailed instructions that must be completed before you can apply for a health card.

  8. You must also attend an hour movie within 30 days of applying for the health card. Check the movie schedule for the time most convenient for you. You do not need to make a reservation. Please arrive on time as seating is limited and doors close promptly at each scheduled movie time.

    Failure to complete the one hour movie within 30 days of applying will require class attendance prior to re-application and paying a full-price reprocessing fee.

  9. You will be given a 30 day temporary card to use until you view the movie.

  10. Your health card needs to be renewed every two or three years depending on the type of card issued.

Duplicate health cards are available to those who have a current Health Card. The fee is $20. Please bring two (2) forms of valid identification, one of which must contain a photo (see valid i.d. list).

X-ray only - $80
Skin Test Only - One Step - $30 **
Skin Test Only - Two Step - $50 **

**Please note: TB skin tests are not administered on Thursdays or on days when they cannot be read in 48 to 72 hours.

Payment Methods Accepted:

Cash, Visa, and MasterCard is accepted in all our locations except Laughlin and Mesquite. All fees paid in Laughlin and Mesquite must be with cash.
