* When the child being adopted was born in the United States,
it is recommended that the adoption be placed on file in the state
where the birth occurred. Please contact the birth state for more
information on filing an adoption.
When a child is born in New Jersey and adopted, the adoptive parent(s)
can place the adoption on file with the State Bureau of Vital Statistics and
Registration.
To place a domestic adoption on file and receive an amended birth
certificate, the following must be provided:
- A completed Report of Adoption form [ PDF 20k]

- A certified copy of the original adoption decree.
- A check or money order for $27 payable to: Treasurer, State of
New Jersey.

Once the adoption is recorded with the New Jersey Bureau of Vital
Statistics and Registration, the original birth certificate is placed
under seal, which can only be broken by court order.
Child born and adopted in a foreign country and readopted
in the U.S.
If the child was born outside the United States and adopted in the
U.S., and at least one of the parents is a U.S. citizen, and the parents
are New Jersey residents at the time of the adoption or at the time
they are placing the adoption on file with the State Bureau of Vital Statistics and
Registration, all of the following
must be provided:
- A No Record of Birth statement from the state where the adoption
occurred. (Not required if the child is adopted in New Jersey.)
- A completed Report of Adoption form [ PDF 20k]

- A certified copy of the original foreign adoption decree and a
certified English translation. **
- A certified copy of the original foreign birth certificate and
a certified English translation. **
- A check or money order for $27 payable to: Treasurer, State of

- Proof of New Jersey residency by providing a copy of one of the

A voter registration card
Driver license
U.S. Federal Census Report of the parents
Current utility bill - telephone, cable, gas, or electric
- Proof that one parent is a U.S. citizen by providing a copy of
one of the following:
U.S. birth certificate
Naturalization Certificate
Foreign birth certificate of a U.S. citizen born abroad
- Proof of the child’s immigration status by providing a copy
of one of the following:
Child’s green card
Child’s foreign passport showing the I-551 stamp
Child’s U.S. citizenship document
** These documents will be retained by the State Bureau of Vital Statistics and
Registration to be placed under seal.

Child born and adopted in a foreign country
